[Clinicopathological and molecular features of two cases of early gastric adenocarcinoma with enteroblastoid differentiation treated by endoscopic submucosal dissection]

Abstract

目的: 探讨早期伴肠母细胞分化胃腺癌(gastric adenocarcinoma with enteroblastic differentiation,GAED)的临床病理、免疫表型和分子遗传学特征。 方法: 收集南京大学医学院附属鼓楼医院2017—2023年经内镜黏膜下剥离术切除确诊的2例早期GAED,采用免疫组织化学、二代测序并结合文献探讨其临床病理和分子学特征。 结果: 2例患者均为男性,年龄分别58和72岁,均发生于胃窦部,且均混合管状腺癌。例1 GAED位于黏膜固有层,例2 GAED侵达黏膜下层,2例均出现典型胞质丰富透亮肿瘤细胞,呈管状、乳头状和巢状排列,免疫组织化学均不同程度表达SALL4、Glypican-3和甲胎蛋白。二代测序例1显示RANBP2、FLT4、CARD11和NOTCH1基因错义突变,ZRSR2移码突变;例2显示SPTA1、FGFR1无义突变,CASP8、TP63、TET2、FBXW7、PIK3C2G、SOX9、SETBP1、BCR错义突变,ANKRD26、TP53剪切突变。 结论: GAED具有较高侵袭性,深入认识其早期临床病理及分子遗传学特征有助于早诊早治,并为进一步机制研究和靶向治疗提供参考。.
